Evidence

The Evidence

This is where the paper trail lives.

Over the course of this case, what began as a request for healthcare generated years of correspondence, complaints, policies, investigations, Freedom of Information requests, internal reviews and escalation through different levels of the NHS and public accountability system.

I am making that documentary record available so that people do not have to rely solely on my account of what happened.

You do not have to take my word for it. Read the evidence.

Wherever possible, this website allows the original emails, letters, policies, responses and investigation documents to speak for themselves.

Much of the ongoing investigations and reporting is still in development; I will try my best to update the folders on an ongoing basis. However, if you think something is missing - please email me and I'll do my best to locate it and share it.

What You Will Find Here

The evidence spans several years and multiple organisations, individuals and accountability processes.

It includes the NHS policies that triggered the original challenge; my correspondence with the NHS; complaint records and responses; the Parliamentary and Health Service Ombudsman investigation; Freedom of Information requests and disclosures; internal inquiries and requests for governance checks; correspondence with senior NHS leadership; and subsequent escalations when I continued asking who was accountable for what had happened.

It also includes correspondence relating to escalation beyond the ICB itself, including approaches to NHS England, senior leadership and the Chair, and local democratic scrutiny in Oxfordshire.

Among the documents is the formal correspondence in which I was told that appropriate processes had been followed, alongside the later evidence and independent findings that allow readers to examine those assurances against what was subsequently established.

Taken individually, many of these documents may look like ordinary administrative correspondence.

Taken together, they tell the story.

Policies. Emails. Complaints. Investigations. FOIs. Governance. Escalations. Answers — and unanswered questions.

Why I Am Publishing the Raw Material

There is inevitably interpretation involved when somebody tells a story this long.

That is precisely why I do not want this website to depend entirely upon my interpretation.

I want journalists, researchers, lawyers, patients, campaigners, regulators and members of the public to be able to go backwards from what I say and examine the underlying material for themselves.

Look at what I asked.

Look at what I was told.

Look at what the policies actually said.

Look at what the investigations later established.

Look at what happened when I asked who was responsible.

Then make up your own mind.

This archive is not here to tell you what to think. It is here to make it much harder for the record to disappear.
