Disclosure and scope: This article combines documented facts with my own analysis as a social psychologist and as the patient whose complaint about access to female sterilisation in Oxfordshire exposed serious failures in NHS policy-making and complaint handling. Where I am offering an interpretation rather than reporting an established causal fact, I say so explicitly.

As a social psychologist, I spend a great deal of time thinking about how ideas spread. We tend to imagine social contagion as something obvious: a slogan goes viral, a political movement suddenly becomes fashionable, or a set of talking points appears everywhere at once. In reality, belief systems often travel much more quietly. They move through professional networks, think tanks, legal arguments, conferences, political relationships, religious communities, donor networks, media framing and repeated language that slowly makes one way of seeing the world feel normal.

That is one reason I have become increasingly concerned by what is happening to reproductive rights in the United States. Since the US Supreme Court overturned Roe v Wade in 2022, abortion access has become dramatically more dependent on where a woman lives. As of September 2026, the Guttmacher Institute reports that 13 US states have total abortion bans in effect and another 28 have gestational bans. The second Trump administration has also reinstated the Mexico City Policy and rescinded Biden-era executive actions intended to protect access to reproductive healthcare. Whatever one's personal views on abortion, this is plainly a large-scale political reorganisation of reproductive healthcare.

Britain is not America. Our legal system is different, our healthcare system is different, and public attitudes are different. But Britain and the United States have also spent more than 80 years describing their relationship as uniquely close. British governments call it the “special relationship”; the US government has described the UK as its closest ally. That closeness is not confined to defence. Ideas, campaign methods, legal strategies, political personnel, philanthropy and culture move across the Atlantic with unusual ease.

So when a well-funded US legal movement that helped shape the American reproductive-rights battles establishes a London presence, increases its UK spending, briefs MPs, supports litigation and builds relationships inside Westminster, I think the right question is not, “Are we becoming America tomorrow?” The right question is: what ideas are being normalised today, who is paying to normalise them, and what happens if we only notice after institutional practice has already shifted?

What does “social contagion” have to do with reproductive rights?

Social contagion does not mean that one country mechanically copies another. It means attitudes, narratives and behavioural models can spread through networks. Political movements learn from each other. Campaigners borrow language that has worked elsewhere. Legal organisations test arguments in one jurisdiction and adapt them to another. Politicians observe which issues mobilise supporters. Media ecosystems repeat frames until they become familiar.

In the United States, the abortion debate was not transformed overnight. It was the result of decades of organising: litigation, judicial appointments, state legislation, donor funding, activist mobilisation, religious campaigning and the construction of an intellectual and legal infrastructure capable of turning a long-term ideological objective into public policy. Dobbs v Jackson Women's Health Organization was the legal endpoint of a much longer political process.

This matters because one of the organisations embedded in that US legal infrastructure — Alliance Defending Freedom, or ADF — now has a substantial British operation.

Who is Alliance Defending Freedom, and why does its UK growth matter?

Alliance Defending Freedom is a US-founded Christian legal advocacy organisation. Its American operation is large: according to its 2024 US tax filing, reported by ProPublica's Nonprofit Explorer, it had revenue of approximately $110.7 million and expenditure of approximately $106.4 million. ADF has participated in major US litigation concerning abortion, religious liberty and LGBT rights. It was involved in the legal movement around Dobbs, the Supreme Court decision that removed the federal constitutional right to abortion.

ADF's international arm has built a presence in European centres of political and legal power, including London, Brussels, Strasbourg, Geneva and Vienna. Its UK entity, ADF International (UK), is a registered charity and company. The Charity Commission describes its charitable purpose as advancing Christianity and promoting Christian principles and ethics.

The financial trajectory is notable. ADF International UK's Charity Commission entry shows income of £1,468,307 and expenditure of £1,404,402 for the year ending 30 June 2025. The Guardian had previously reported that its spending rose from roughly £392,000 in 2020 to almost £1 million in the year to June 2023. This is no longer a tiny overseas campaign office operating at the edge of British political life. It is a seven-figure organisation with staff, lawyers, parliamentary relationships and an explicit advocacy strategy.

What does ADF say it is doing?

ADF presents its work as human-rights advocacy centred on religious freedom, freedom of speech, parental rights and protection of life. In Britain it has repeatedly framed disputes around abortion-clinic “buffer zones” or “safe access zones” as questions of free expression and religious freedom. It has supported or represented people accused of breaching restrictions around abortion clinics, and it has opposed efforts to decriminalise abortion.

That framing deserves to be represented accurately. ADF does not publicly describe itself as an organisation trying to import an American culture war. It says it is defending fundamental freedoms and the right to life. People who share those values may regard its work as principled civil-liberties advocacy.

Critics see something different: a sophisticated legal organisation using rights-based language to pursue an agenda that has historically included restricting abortion access and opposing LGBT rights. Those concerns are not based only on rhetoric. They arise from ADF's litigation record, its US political relationships and its expanding European presence.

How close has ADF UK come to British political power?

The answer is: close enough that it deserves serious public attention.

In 2024, The Guardian reported that ADF International UK had been producing policy briefings and legal analysis for MPs, including ahead of votes concerning buffer zones around abortion clinics. The newspaper also reported that ADF had been appointed as a stakeholder to a parliamentary group concerning freedom of religion or belief, and that in September 2023 it paid travel and accommodation costs for Fiona Bruce MP, then the Prime Minister's Special Envoy for Freedom of Religion or Belief, to attend a conference.

In April 2025, Baroness Bennett asked the Government what assessment it had made of the impact of lobbying by overseas-funded organisations such as ADF on UK human rights and democracy. The Government did not announce a specific investigation into ADF. Its answer instead pointed to ministerial transparency rules, parliamentary registers and rules governing all-party parliamentary groups.

That response is revealing in its own way. The existence of foreign-linked ideological advocacy inside Westminster is not hypothetical; it has become sufficiently visible to generate parliamentary questions about transparency and influence.

Is ADF the whole story?

No. Focusing exclusively on ADF would make the picture misleadingly simple. Britain's anti-abortion movement includes longstanding domestic organisations, parliamentary networks, religious groups, campaign organisations and newer transnational actors. Some are primarily British and funded largely through British supporters. Some have international ideological or organisational connections. They do not all share the same tactics or every policy position.

Right To Life UK: a direct parliamentary bridge

Right To Life UK is a British registered charity focused on abortion, assisted suicide and embryo research. It describes its mission as protecting human life from conception to natural death and says it works through education, politics, media and a network of supporters across the UK.

Its political role is unusually concrete. The official Parliamentary Register records that Right To Life UK acts as secretariat to the All-Party Parliamentary Pro-Life Group. As of February 2026, the APPG was chaired by DUP MP Carla Lockhart, with Baroness Eaton, Sir Desmond Swayne and Baroness Ritchie of Downpatrick among its officers. Right To Life UK itself says its public-affairs team supplies research, briefings and policy analysis to MPs and peers and has direct relationships with parliamentarians across parties.

Its financial scale is also significant. Charity Commission figures for the year ending March 2025 record income of £1,164,156 and expenditure of £1,231,188. Right To Life UK says it receives no government funding and is funded by supporter donations.

This is important because influence does not require secret meetings or conspiracies. An organisation that drafts briefings, provides an APPG secretariat, mobilises constituents to contact MPs, tracks voting records and maintains relationships with parliamentarians is describing ordinary, organised political advocacy. The democratic question is not whether advocacy should exist. Of course it should. The question is whether the public understands who is doing it, how well resourced they are, and how their goals fit into a broader ecosystem.

SPUC, 40 Days for Life and clinic-focused campaigning

The Society for the Protection of Unborn Children (SPUC) is one of Britain's oldest anti-abortion organisations. It has campaigned for decades against abortion and on related bioethical issues. Separately, the US-originated 40 Days for Life model — prayer vigils and activism around abortion clinics — has been adopted in Britain. OpenDemocracy has reported that British anti-abortion organiser Robert Colquhoun described being inspired by US counterparts and was credited by 40 Days for Life with helping export the model into the UK.

Again, the point is not that every British campaign is controlled from America. The more interesting and better-supported conclusion is that tactics, legal theories and movement know-how cross borders. Domestic organisations can remain domestically run while still participating in an international political culture.

How much money is moving through the wider European “anti-rights” ecosystem?

We should be careful with labels here. “Anti-rights” and “anti-gender” are terms used by organisations that monitor movements opposing abortion rights, LGBT rights, sex education and some gender-equality policies. The groups being described often reject those labels and instead describe themselves as pro-life, pro-family or defenders of religious freedom.

With that caveat, the scale documented by the European Parliamentary Forum for Sexual and Reproductive Rights (EPF) is substantial. Its 2021 report, Tip of the Iceberg, identified approximately $707 million in spending by a sample of organisations active across Europe between 2009 and 2018, including about $81 million originating from US actors. EPF's 2025 follow-up, The Next Wave, reported $1.18 billion in spending by anti-rights and anti-gender initiatives in Europe between 2019 and 2023.

These numbers do not mean that $1.18 billion was spent on British abortion lobbying, nor that all organisations in the dataset have identical aims. They do show that the European political environment around sexuality, reproduction and gender now includes a large, professionalised, transnational advocacy sector.

Why does the American example matter now?

Because the American movement demonstrates what long-term infrastructure can accomplish.

In June 2022, the Supreme Court decided Dobbs and overturned the constitutional framework created by Roe v Wade. By September 2026, Guttmacher counted 13 states with total abortion bans and 28 more with gestational bans. The legal conflict has also moved to medication abortion. Just this month, a federal appeals court has been considering another attempt by Republican-led states to restrict access to mifepristone, the drug used in most US abortions.

At federal level, President Trump in January 2025 ordered enforcement of the Hyde Amendment as administration policy and revoked two Biden executive orders designed to protect access to reproductive healthcare. On the same day, he reinstated the Mexico City Policy for overseas health assistance.

None of those developments proves that Britain will follow the same path. But they demonstrate that reproductive rights that appear politically settled can become unsettled when sufficiently organised movements acquire legal, political and institutional leverage.

But what does any of this have to do with female sterilisation?

This is where I want to be exceptionally clear.

I have found no evidence that ADF, Right To Life UK, SPUC or any other anti-abortion organisation caused the NHS restriction that affected my access to female sterilisation. I have also found no evidence that ADF lobbied NHS England to weaken national wording about the funding of female sterilisation.

I will not manufacture that connection merely because it would make a dramatic story.

My concern is about something broader: the political environment in which women's reproductive autonomy is interpreted. Reproductive freedom does not begin and end with abortion. It includes access to contraception, informed consent, the ability to continue a pregnancy, the ability to end one within the law, fertility care, and — for adults who do not want children — the ability to make a considered permanent choice not to become pregnant.

When a political culture becomes increasingly comfortable with the idea that institutions may override women's reproductive preferences “for their own good”, “for the unborn”, “because they may regret it”, “because another method is preferable”, or because access should depend on local policy, I pay attention. The arguments are not identical, but the underlying question of who gets final authority over reproductive decisions is connected.

What changed on the NHS website — and why do I consider it monumental?

The current NHS England information on female sterilisation says:

“Sterilisation may be available free on the NHS, or you may need to pay for your treatment.”

Read that next to the NHS's general contraception information, which states that contraception services are free and confidential on the NHS, and next to the NHS Constitution's principle that access to NHS services is based on clinical need rather than ability to pay.

The wording matters because female sterilisation is itself listed by the NHS as a method of contraception. It is more than 99% effective and does not affect hormones. Yet the national patient-facing page now explicitly tells women that this particular contraceptive option may not be funded where they live.

Older NHS-facing and NHS-derived information presented contraception much more straightforwardly as free on the NHS, and Sexual Health London material reviewed in 2020 stated directly that female sterilisation was available free on the NHS. The present national wording is therefore materially weaker than the expectation historically created around NHS contraception.

For me, this is not a cosmetic website edit. It is a public acknowledgement of a postcode lottery.

What does a postcode lottery look like in practice?

It means two women with the same reproductive wishes, similar clinical circumstances and equal capacity to provide informed consent may encounter different pathways because they live under different commissioners. One may be referred. Another may be required to try long-acting reversible contraception first. Another may discover that the service is not routinely commissioned. Another may be told she can pay privately.

That is precisely the kind of local variation that my own case forced me to confront. The Oxfordshire policy applied to me required women seeking sterilisation to use long-acting reversible contraception first, unless exceptions applied. My complaint was not simply that I disliked the answer. I challenged the evidential, equality and governance basis of a policy that restricted a permanent reproductive choice.

The Parliamentary and Health Service Ombudsman later found failures in the way the policy and my complaint had been handled. Yet the broader problem remains: a national health service can still allow local commissioning decisions to determine whether a fundamental reproductive option is practically available.

Is local commissioning automatically a violation of women's rights?

No. The NHS has finite resources, and Integrated Care Boards make local commissioning decisions across many services. Different clinical needs and local service structures can legitimately produce variation.

But that cannot be the end of the conversation. NHS England and ICBs also have statutory duties concerning equality and health inequalities. The NHS Constitution states that the service should be comprehensive, available to all, and that patients should not be refused access on unreasonable grounds. It also says care should reflect patients' needs and preferences.

So the real question is not whether any local variation may exist. It is: how much variation is acceptable when the service in question concerns bodily autonomy and reproductive decision-making, and what evidence must commissioners produce before restricting it?

Could Britain lose reproductive rights without ever announcing that it is doing so?

That is the question that worries me most.

Rights do not always disappear through a single dramatic Act of Parliament. Sometimes they become harder to exercise. A service stops being routinely commissioned. Eligibility criteria narrow. Waiting lists become prohibitive. A procedure remains technically legal but practically inaccessible. National wording shifts from “available” to “may be available”. Patients are redirected to private payment. The formal right survives while meaningful access erodes.

From a social-psychological perspective, this is also how norm change can occur: not necessarily by persuading everyone that a right should disappear, but by gradually changing what people accept as normal.

That is why I continue pressing the NHS for accountability long after my own individual complaint could have ended. I do not need to prove that an American organisation wrote an Oxfordshire sterilisation policy in order to be concerned about the wider environment. I need to show, carefully and document by document, where women's choices are being narrowed, who is lobbying political institutions on reproductive issues, what ideas are crossing borders, and whether the safeguards that are meant to protect equality and patient autonomy are actually working.

So, is Britain importing America's reproductive-rights culture war?

Not wholesale. Not inevitably. And not in the simplistic sense that Washington gives instructions and Westminster follows them.

But there is strong evidence that American organisations, money, legal strategies and campaign models are operating inside the British and European political environment. There is strong evidence that ADF's UK spending and Westminster activity have grown. There is strong evidence of organised parliamentary advocacy by domestic anti-abortion organisations. And there is undeniable evidence that the American reproductive-rights settlement once regarded as constitutionally secure was dismantled after decades of organised political and legal work.

For me, those facts justify vigilance, not conspiracy.

And when, at the same time, the national NHS website tells women that permanent contraception “may” be funded or they “may” have to pay, I think we should ask harder questions about what reproductive freedom actually means in Britain.

For a closer investigation of the organisations, funding, parliamentary relationships, litigation strategies and political machinery behind this movement, read my companion article: The American Anti-Abortion Movement Is Spending Money in Britain. What Does It Want?

Editorial note: I distinguish throughout between documented lobbying and funding, criticism made by reproductive-rights organisations, and my own interpretation of the risks posed by gradual restrictions on reproductive autonomy. I have found no evidence that ADF or another anti-abortion organisation caused the NHS female-sterilisation commissioning restriction discussed in my case.

Disclosure and scope: I am writing this as a social psychologist and as a woman whose challenge to an NHS female-sterilisation policy became a prolonged accountability battle. This is an evidence-led investigation. It does not allege that the organisations discussed below caused the NHS policy in my case; where no such evidence exists, I say so.

As a social psychologist, I am fascinated by the moment when an idea stops looking fringe and starts looking normal. Social contagion is often misunderstood as imitation: one person copies another, one country copies another. But ideological contagion is usually more sophisticated. A belief travels with a network. It gains lawyers, donors, spokespeople, parliamentary contacts, research papers, campaign language, sympathetic media platforms and repeated opportunities to define what the argument is supposedly “about”.

This is why the United States matters to me when I look at women's reproductive rights in Britain.

America and Britain have what both governments still call a “special relationship”. The two states cooperate unusually closely in diplomacy, security, economics, technology and culture. Their political movements also watch each other. Campaign tactics cross the Atlantic. Think tanks quote one another. Lawyers share arguments. Conferences connect activists. Donors fund international work. Language developed in one political system can become useful in another.

At the same time, the United States has undergone one of the most dramatic reversals of reproductive-rights protection in the modern democratic world. Roe v Wade was overturned in 2022. By September 2026, 13 states had total abortion bans and 28 more had gestational bans. Federal policy under President Trump has again moved in an anti-abortion direction, including reinstatement of the Mexico City Policy and reversal of Biden-era reproductive-health executive actions.

So I wanted to ask a more forensic question than “Could this happen here?”

Who is already working here, how much money is involved, who has access to Parliament, which legal and political strategies are being used, and what do these organisations themselves say they want?

This is the narrower companion to my broader article, Is Britain Importing America's Reproductive-Rights Culture War?, which looks at social contagion, the US-UK relationship and why I connect this political environment to vigilance over women's NHS reproductive choices.

Start with the money: how large is ADF's operation?

Alliance Defending Freedom began in the United States and has become a major Christian legal advocacy organisation. Its US financial scale alone makes it different from the picture many British readers may have of “pro-life activism” as a collection of church volunteers and local campaigners.

ProPublica's Nonprofit Explorer reports that Alliance Defending Freedom's US Form 990 for the fiscal year ending June 2024 recorded revenue of $110,713,980, expenditure of $106,394,230 and net assets of roughly $90.2 million. More than $103 million of its revenue was reported as contributions.

ADF's international operation has offices in strategic European and international centres. The UK entity was incorporated in 2015 and registered with the Charity Commission in 2017. Its registered charitable objects are to advance Christianity by promoting Christian principles and ethics and supporting Christians to live and worship in accordance with them.

The British financial figures have climbed sharply. The Charity Commission records ADF International UK income of £1,468,307 for the year ending 30 June 2025, of which approximately £1.41 million was donations and legacies. Expenditure was £1,404,402, with approximately £1.39 million recorded as charitable expenditure.

For perspective, The Guardian reported that its expenditure was about £392,556 in 2020 and nearly £1 million by the year ending June 2023. That is a substantial expansion in a short period.

Where does ADF UK's money come from?

This is where transparency becomes complicated.

Charity Commission accounts tell the public how much the UK charity receives and spends, and the broad accounting categories involved. They do not provide a public list of every individual donor. ADF in the United States has defended donor privacy in litigation and does not publish a comprehensive donor list. Critics therefore use the phrase “dark money” when discussing parts of the US Christian-right funding ecosystem: not necessarily because the money is illegal, but because the original private sources can be difficult for the public to identify.

It is important not to overstate what that means. A lack of public donor names is not proof of wrongdoing. Charities and advocacy organisations across the political spectrum may lawfully protect donor privacy. The public-interest issue is different: when an organisation is trying to influence law and policy in another country, how much should citizens be able to know about the financial interests sustaining that influence?

That question has been asked at European level for years. The European Parliamentary Forum for Sexual and Reproductive Rights reported in 2021 that it had identified $707.2 million in spending by 54 organisations associated with “anti-gender” activity across Europe from 2009 to 2018, including $81.3 million from US-based actors. Its 2025 report said spending across a larger anti-rights ecosystem totalled $1.18 billion between 2019 and 2023.

Those are advocacy-sector estimates and should be read as such. They include organisations working on a range of issues beyond abortion. They nevertheless establish something important: organised opposition to sexual and reproductive rights in Europe is not financially marginal.

What does ADF actually want?

To answer fairly, start with ADF's own framing.

ADF describes itself as defending fundamental freedoms, especially religious freedom and freedom of expression, and protecting life. Its UK legal work frequently focuses on the ability of Christians to express beliefs in public or act according to conscience. Around abortion, it has opposed decriminalisation and challenged or criticised restrictions on speech and prayer near abortion clinics.

That civil-liberties framing is politically powerful because it changes the apparent question. Instead of “Should campaigners be allowed to confront or influence women entering abortion services?”, the issue becomes “Should the state criminalise silent prayer?” Instead of “Should abortion law be liberalised?”, it becomes “Should unborn life lose legal protection?”

This is not unique to ADF. Successful political advocacy often depends on selecting the moral frame through which everyone else is forced to argue.

Why do critics focus on ADF's American record?

Because the organisation's US legal work places its British arguments in a larger context. ADF was part of the legal ecosystem that spent years challenging abortion rights and participated in the Dobbs litigation that ultimately overturned Roe. It has also litigated extensively on LGBT rights, contraception-related employer mandates and religious exemptions.

Critics therefore argue that apparently narrow British free-speech cases should not be viewed in isolation from the organisation's wider strategic goals. ADF rejects characterisations of itself as extremist and presents its work as principled human-rights advocacy.

For readers, the responsible conclusion is not that every ADF case is secretly “about” banning abortion. It is that an organisation's full litigation history is relevant context when assessing the long-term policy direction of its advocacy.

What is ADF doing in Westminster?

ADF's own annual-report language and investigative reporting indicate a deliberate focus on decision-makers. The Guardian reported in 2024 that the organisation had sought to engage “significant decision-makers” and had provided briefing material and legal analysis to MPs before votes on buffer zones around abortion clinics.

It had also become a stakeholder in parliamentary work on freedom of religion or belief. In 2023, it paid travel and hotel costs for Fiona Bruce MP, then the Prime Minister's Special Envoy for Freedom of Religion or Belief, to attend a conference. The relevant point is not that paying declared travel expenses proves improper influence. It does not. The point is that the organisation had reached a level of political proximity where such interactions occurred.

By April 2025, the issue had reached the House of Lords. Baroness Bennett asked the Government what assessment it had made of the impact on democracy and human rights of lobbying by overseas-funded groups “such as the Alliance Defending Freedom”. The Cabinet Office response stressed existing transparency mechanisms rather than offering a substantive assessment of ADF's impact.

Who else has structured access to Parliament?

Right To Life UK provides perhaps the clearest example of organised domestic anti-abortion access.

The official parliamentary register states that Right To Life UK acts as secretariat to the All-Party Parliamentary Pro-Life Group. In February 2026, the APPG listed Carla Lockhart MP as chair and registered contact, with Baroness Eaton as co-chair and Sir Desmond Swayne and Baroness Ritchie as officers.

Right To Life UK's own website is explicit about what this relationship is for. It says its public-affairs team provides research, briefings and expert policy analysis to MPs and peers, builds direct relationships with parliamentarians and uses campaign tools enabling supporters to contact their MPs with pro-life messaging. It also operates a voting-record platform showing where MPs stand on “life issues”.

This is lobbying in the broad democratic sense of organised efforts to influence representatives, even though UK law uses narrower technical definitions for some lobbying regulation. It is visible, structured and ongoing.

The Charity Commission records income of £1,164,156 and expenditure of £1,231,188 for the Right To Life Charitable Trust in the year ending March 2025. The organisation says it receives no government funding and depends on supporter donations.

What policies are these organisations pushing?

There is not one single manifesto shared by every anti-abortion organisation. But several recurring objectives are visible.

1. Prevent further liberalisation of abortion law

Right To Life UK and ADF have both opposed moves that would decriminalise aspects of abortion law. Right To Life UK routinely mobilises supporters around parliamentary amendments and votes, urging them to contact MPs. Its stated mission is to protect human life from conception.

2. Preserve or expand legal recognition of fetal interests

Campaign groups support amendments that would narrow circumstances in which abortion is lawful or change gestational limits. Right To Life UK has, for example, produced parliamentary briefings seeking equalisation of the abortion time limit for pregnancies involving serious fetal disability.

3. Contest buffer zones and safe-access zones around clinics

ADF has made this one of its most visible UK legal fronts. Supporters frame the issue around prayer and speech. Abortion providers and reproductive-rights organisations frame buffer zones as protection for patients and staff from harassment, intimidation and unwanted influence at a medically and emotionally sensitive moment.

Both sets of rights can be described sincerely. The policy conflict lies in where society draws the boundary between political expression and protected access to healthcare.

4. Strengthen conscience protections

Christian legal organisations often argue that doctors, nurses and other professionals should not be compelled to participate in practices they consider morally wrong. Reproductive-rights advocates worry that broad conscience protections can reduce practical access to lawful healthcare. This is another area where the same proposal can be framed either as protecting an individual professional or restricting a patient's access.

5. Build public and parliamentary opposition through mobilisation

Right To Life UK openly describes constituency mobilisation as part of its strategy. Its online tools make contacting MPs easier. Campaigns are timed around votes and legislative threats. This is a classic political organising method: convert a national supporter base into local pressure that MPs experience as constituent opinion.

What role do litigation and “lawfare” play?

Litigation is one of the most important differences between a modern professional advocacy network and a traditional protest movement.

A protest tries to persuade the public or pressure politicians. Strategic litigation tries to change what the law means. A successful case can alter rules far beyond the individuals named in the lawsuit. Even an unsuccessful case can create publicity, attract donors, develop legal arguments and establish a pipeline of future cases.

ADF is particularly experienced in this model. Its international operation has intervened in courts and human-rights institutions across Europe. OpenDemocracy and EPF have documented the importance of strategic legal advocacy within US Christian-right activity in Europe.

The US experience shows why this matters. The overturning of Roe was not produced by one election or one protest. It required decades of litigation, scholarship, political campaigning, judicial selection and institutional development. Once a movement has a legal infrastructure, political goals that once seemed impossible can become justiciable questions waiting for the right court.

Who shares the same political circles?

Networks matter, but they must be described carefully. Sharing a conference, parliamentary group or ideological objective does not make organisations identical, and association is not proof that one actor controls another.

Still, several overlapping circles are visible.

Fiona Bruce, a former Conservative MP and Prime Ministerial Special Envoy for Freedom of Religion or Belief, appeared in ADF-linked activity and had travel expenses paid by ADF International UK for a 2023 conference, according to reporting based on the charity's accounts.

Carla Lockhart MP chairs the All-Party Parliamentary Pro-Life Group, for which Right To Life UK is secretariat.

Right To Life UK maintains relationships across party lines and explicitly says it briefs MPs and peers.

40 Days for Life, an American-origin anti-abortion campaign, has supplied a model for clinic-focused activism used in Britain.

Alliance Defending Freedom connects the British scene to a much larger US and international legal network. In America, former ADF lawyer Mike Johnson became Speaker of the House of Representatives, illustrating the organisation's proximity to senior US conservative politics even though he is not part of ADF UK's governance.

The important pattern is not a secret command structure. It is an ecosystem in which legal organisations, parliamentary advocates, grassroots campaigners, donors, religious networks and sympathetic politicians reinforce one another.

Is this “foreign interference”?

That phrase is too strong for the evidence discussed here if it implies covert or unlawful state interference.

ADF International UK is a legally registered British charity and company. Its public advocacy, litigation and contact with politicians take place through lawful channels. The UK Government has pointed to disclosure rules governing ministerial meetings, parliamentarians and APPGs.

The better issue is foreign-funded or transnational political influence. Democracies routinely permit international civil society organisations to campaign on human rights, climate, trade, religion and many other issues. Reproductive rights should not be treated differently merely because the advocacy comes from the political right.

But transparency should be strong enough for citizens to understand the scale, funding and access of actors trying to shape law. That principle should apply whether the organisation is anti-abortion, pro-choice, environmentalist, corporate or anything else.

What is the connection to the NHS?

Here is the most important evidential boundary in this article.

I have not found evidence that ADF International UK, Right To Life UK or SPUC directly influenced NHS England's policy or public wording on female sterilisation. I have not found evidence that these organisations designed the Oxfordshire policy I challenged.

The political influence documented above is primarily around abortion, assisted dying, embryo issues, freedom of religion, speech and related culture-war questions. That is already significant. There is no need to invent an unsupported sterilisation conspiracy.

Why, then, am I writing about them on a site born from my NHS sterilisation battle?

Because my concern is reproductive autonomy as a system. A political movement does not need to target my exact medical procedure before it becomes relevant to the environment in which women's reproductive choices are valued, funded or restricted.

Why does the NHS wording on female sterilisation belong in this conversation?

What you'd see today in September 2026 is that NHS England's current page on female sterilisation says that the procedure “may be available free on the NHS, or you may need to pay for your treatment.” However, this is a recent change of the past year. It changed during the Ombudsman investigation of my case with the ICB Thames Valley - before this change - NHS England's site used to state that Female Sterilisation is funded on the NHS. That's not a small change that shifts access to the procedure.

So, yes, that is extraordinary word changing for a contraceptive method on a national health-service website.

The same NHS website states elsewhere that contraception services are free and confidential on the NHS. Female sterilisation appears in the NHS list of contraceptive methods. The NHS also states that sterilisation is more than 99% effective and does not affect hormones.

Older NHS-facing information presented contraception as free more categorically, while NHS-partner Sexual Health London information reviewed in 2020 stated that female sterilisation was available free on the NHS. The current national sentence therefore does something important: it normalises the possibility that a woman seeking permanent contraception may be told the NHS will not fund it and that private payment is the alternative.

I regard that as an erosion of reproductive access and an official acknowledgement of postcode-based inequality.

Does local commissioning explain the wording?

Almost certainly it explains much of the practical variation. Sterilisation sits outside the open-access contraception services that local authorities are legally required to provide, and secondary-care commissioning can depend on NHS commissioners. Different ICBs therefore can have different pathways, thresholds and funding arrangements.

But “local commissioning” describes the mechanism; it does not answer the ethical question.

Why should a competent woman's ability to obtain a permanent contraceptive procedure through the NHS differ materially by postcode? What evidence justifies requiring her to try another method first? Are equality implications examined? How are side effects, patient preferences and the cumulative burden of long-term contraception weighed? If a man can obtain a locally commissioned vasectomy through a relatively simple route while a woman faces additional thresholds, has sex equality been properly considered?

These are the kinds of questions my complaint forced into the open.

What should we watch for if reproductive rights are eroding?

I would not look only for dramatic announcements such as “Britain bans X”. Institutional erosion is often incremental - your typical slow boil of the frog situation.

• A legal service remains technically available but becomes difficult to access.
• National guidance becomes less definite about entitlement.
• Commissioning thresholds spread from one region to another.
• Medical choice is reframed as something patients must earn after trying institutionally preferred alternatives.
• Lobbying networks expand while transparency remains weak.
• Rights debates are reframed so that restrictions appear to be merely questions of cost, conscience, safety or administrative discretion.
• People become accustomed to paying privately for something they once expected the NHS to provide.

None of these signs alone proves ideological capture. Together, however, they tell us where democratic and institutional scrutiny should be concentrated.

What can Britain learn from the United States without becoming hysterical about it?

First, rights are not self-enforcing. A right can exist in statute or precedent while access deteriorates in practice.

Second, organised minorities can have disproportionate influence when they build durable institutions. A motivated network with lawyers, policy staff, donors and relationships can shape a debate more effectively than a much larger but less organised public majority.

Third, language matters. “Religious freedom”, “women's safety”, “conscience”, “taxpayer protection”, “local choice” and “clinical discretion” can all describe legitimate concerns. They can also become vehicles through which access is narrowed. We should evaluate the practical effect of a policy, not only the label attached to it.

Fourth, transparency matters before a crisis, not after it. Once the public finally notices that an institutional norm has shifted, rebuilding the old consensus can be much harder than defending it earlier.

What do I think this movement ultimately wants?

There is no single answer because the actors differ.

Right To Life UK tells us directly that it wants human life protected from conception to natural death and works politically against liberalisation of abortion and assisted dying. ADF says it wants protection for life alongside stronger religious-freedom, conscience and free-speech rights. SPUC campaigns for legal protection of unborn children and against abortion. 40 Days for Life aims to end abortion through prayer, fasting, outreach and clinic-focused vigils.

If we take those organisations at their own words, the shared direction around abortion is clear: fewer abortions, stronger legal protection for fetal life, and resistance to reforms that make abortion easier to access or less criminalised.

Critics go further and argue that the international movement forms part of a wider campaign against sexual and reproductive rights, LGBT equality and aspects of gender equality. EPF's reports are the most detailed attempt to map that wider European ecosystem.

My own concern is narrower than claiming all of these actors are one conspiracy. I am concerned that once institutions become comfortable restricting one domain of reproductive autonomy, we should scrutinise restrictions in adjacent domains more carefully — particularly when women are told that the reproductive choice they want is not locally funded.

Why am I still pushing the NHS for accountability?

Because accountability is preventive.

My sterilisation case could be treated as an administrative dispute from 2022: one woman, one old Oxfordshire policy, one complaint. I refuse to see it that narrowly.

The failures exposed by the case raise questions about how reproductive-health policies are evidenced, who approves them, how equality is assessed, how patients challenge them and what happens when an Ombudsman finds maladministration. Those governance questions become more important — not less — in a political environment where organised groups are investing heavily in changing the boundaries of reproductive law.

I cannot tell you that ADF changed my NHS policy. The evidence does not say that.

I can tell you that a powerful US organisation deeply involved in America's reproductive-rights battles now spends more than £1 million a year through its UK arm; that it has briefed MPs and engaged senior political figures; that a domestic anti-abortion charity with more than £1 million annual income acts as secretariat to a parliamentary pro-life group; that transnational anti-rights funding in Europe is measured in hundreds of millions; and that the NHS now publicly warns women that female sterilisation may not be free where they live.

Those facts do not need embellishment. They need scrutiny.

The question Britain should ask before it is too late

The most dangerous assumption in any democracy is that “it couldn't happen here”. History rarely repeats in identical form. Britain does not need to recreate the American abortion system in order to absorb American political methods. It does not need a Supreme Court reversal to experience a gradual narrowing of reproductive access. It does not need one master organisation coordinating everything for multiple groups to push political norms in the same general direction.

Social contagion works because people adapt ideas to their own environment.

That is why I am interested in the organisations, the money, the relationships and the language now — while there is still time to ask ordinary democratic questions about them.
